Seeking an experienced Nurse Practitioner to perform home health risk assessments in and around Champaign County, OH, including Urbana, St. Paris, and Mechanicsburg. This per diem position offers flexible scheduling between Monday and Sunday, allowing you to create your own shifts and determine your hourly commitments each week or bi-weekly. You will visit Medicare Advantage plan members in their homes, conducting thorough risk assessments that support quality patient care.

Key Responsibilities:

Perform comprehensive annual risk assessments in patients homes (approximately 45 minutes per visit) Evaluate medical histories, current conditions, and medication lists Educate members on the management of their health conditions and the proper use of medications Identify care gaps and recommend steps for resolution to improve overall patient health Visits may involve annual assessments or post-discharge check-ins No medication prescribing or treatment planning required

Qualifications:

Active Nurse Practitioner License in the state of Ohio Current CPR (BLS) certification Board Certification (AANP or ANCC) Master of Science in Nursing (MSN) from an accredited program At least 6 months of current Nurse Practitioner experience preferred Take advantage of pay rates of $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
