Responsibilities:

Perform annual and post-discharge home health risk assessments for Medicare Advantage members Review medical history, current health conditions, and medication history during 45-minute evaluations Identify gaps in care and suggest recommendations for resolution Educate patients regarding their medical conditions and medications No need for creating treatment plans or prescribing medication

Qualifications:

Active Nurse Practitioner license in Ohio Board Certification via AANP or ANCC Current CPR (BLS) certification MSN from an accredited school of nursing At least 6 months of recent NP experience preferred
