Job Description

Overview We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care. As a Home Health RN, you will have the opportunity to make a significant impact on the lives of patients and their families. Responsibilities Conduct comprehensive assessments of patients' health status and needs. Develop and implement individualized care plans based on patient assessments. Administer medications and treatments as prescribed, including IV infusions. Provide education to patients and families regarding health management, medications, and discharge planning. Document all patient interactions and medical information accurately in medical records. Collaborate with interdisciplinary teams to coordinate care for patients with diverse needs, including behavioral health. Perform triage for incoming patient inquiries and concerns. Offer support in family planning and toddler care as needed. Maintain compliance with all regulatory requirements and best practices in home health care. Experience Current Registered Nurse (RN) license in the state of practice. Previous experience in home health, ER, or clinic settings is preferred. Proficiency in medical documentation practices is essential. Experience with medication administration and IV infusion techniques is highly desirable. Familiarity with discharge planning processes is beneficial. Strong interpersonal skills to effectively communicate with patients, families, and healthcare teams. Pay:
$30.00 - $40.00 per hour
