Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage members, typically lasting 45 minutes per visit Evaluate patients medical history, current conditions, and medication usage Educate patients and families about health conditions and appropriate medication usage Identify gaps in care, highlighting opportunities for intervention and recommending resolution strategies Visits may include annual assessments or post-discharge evaluations This role does not require prescribing medications or developing treatment plans

Qualifications:

Active Nurse Practitioner License in Ohio Current CPR certification (BLS) Board certification through AANP or

ANCC MSN
