This opportunity is tailored for an experienced Nurse Practitioner with a background in home health to perform home-based clinical assessments throughout Champaign County. In this per diem position, you will conduct detailed home health risk evaluations, supporting Medicare Advantage members. Your visits will range from annual health assessments to post-discharge check-ins and will be scheduled in a variety of locations, including Urbana, St. Paris, and Mechanicsburg.

Key Responsibilities:

Perform annual home health risk assessments for assigned Medicare Advantage plan members Review and evaluate both prior and current medical conditions as well as medication histories Deliver education to patients concerning medical issues and prescribed medications Identify and document care gaps, offering recommendations to improve patient outcomes Focus on assessments only; treatment planning and prescribing are not required

Qualifications:

Current, unrestricted Nurse Practitioner license within the state of Ohio Board Certification via AANP or ANCC Active CPR certification (BLS) Masters degree in Nursing (MSN) from an accredited institution At least six months of hands-on NP experience preferred This position offers compensation of $80-$90 per assessment, with additional mileage reimbursement and administrative time included.
