We are looking for a compassionate, dependable Registered Nurse (RN) who wants more autonomy, meaningful patient relationships, and an alternative to the traditional hospital environment. This position is ideal for an RN who enjoys managing their own day, developing relationships with patients and families, and providing high-quality care in the comfort of the patient's home. Provide skilled hospice nursing visits in patients' homes, assisted living, or nursing facilities as assigned. Manage pain and end-of-life symptoms (nausea, dyspnea, agitation, anxiety), adjusting care in collaboration with the hospice physician. Provide wound care, medication management, and other comfort-focused skilled nursing interventions per the plan of care. Educate and support patients and families on disease progression, the dying process, and symptom management. Coordinate with the interdisciplinary team (physician, social worker, chaplain, home health aide, volunteers, bereavement counselor) to meet patients' and families' physical, emotional, and spiritual needs. Communicate condition changes to the physician and team, including signs of active dying, and provide on-call/after-hours support and death pronouncement as needed. Complete timely clinical documentation supporting hospice eligibility, and participate in IDG care planning meetings. Maintain compliance with Medicare hospice regulations, HIPAA, and agency policies. Qualifications Current and unrestricted Ohio Registered Nurse (RN) license . Current BLS/CPR certification. At least 1 year of hospice experience. Experience in home care, hospitals, skilled nursing, rehabilitation, assisted living, hospice, or other clinical environments is welcomed. Strong assessment and critical-thinking skills. Excellent communication and patient-education skills. Comfortable working independently in patients' homes. Reliable transportation and ability to travel within the assigned East Cleveland territory. Strong organizational and time-management skills. Comfortable using electronic medical record systems and mobile technology.
