Responsibilities:

Conduct comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members. Review and document each patients prior and current medical history, conditions, and medication regimen. Educate patients on their health conditions and medication protocols. Identify and document gaps in care, providing clear recommendations for follow-up and resolution. Focus exclusively on risk assessment; no treatment planning or medication prescribing required. Complete assessments within a roughly 45-minute timeframe per visit.

Desired Qualifications:

Active Nurse Practitioner license in Ohio. Board Certification through the AANP or ANCC. Master of Science in Nursing (MSN) from an accredited school. Current CPR/BLS certification. At least 6 months of recent NP experience preferred. Strong communication abilities and a commitment to delivering thorough, patient-focused assessments.
