An exciting per diem opportunity is available for an experienced Nurse Practitioner with expertise in home health. This position focuses on conducting home health risk assessments for Medicare Advantage plan members residing throughout Fulton County, including Wauseon, Archbold, and Swanton, OH. Each visit averages 45 minutes and may involve annual health assessments or post-discharge evaluations. Your clinical knowledge will help identify health risks and gaps in carewithout the responsibility for creating treatment plans or prescribing medication.

Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members Review and evaluate patients current and previous medical history, ongoing conditions, and medication profile Provide patient education regarding health conditions and prescribed medications Detect gaps in care, making recommendations for appropriate interventions Complete necessary documentation and communicate findings as needed

Qualifications:

Active Nurse Practitioner license in Ohio Board certification through AANP or ANCC Current CPR certification (BLS) MSN degree earned from an accredited nursing program At least 6 months of recent Nurse Practitioner experience (preferred) In this rewarding role, you will receive $80-$90 per evaluation, plus mileage reimbursement and compensation for administrative time.
