Responsibilities:

Complete comprehensive annual home health risk assessments, typically 45 minutes per visit Review and evaluate patients current and past medical history, conditions, and medication usage Educate patients about their medical conditions and medications to promote understanding and adherence Identify gaps in care, providing actionable recommendations to improve health outcomes Visits may involve a variety of assessment types, including annual checkups and post-discharge follow-ups No prescribing or treatment plan development requiredfocus solely on evaluation and education

Qualifications:

Active Nurse Practitioner license in Ohio Board certified through AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution Current BLS/CPR certification Minimum 6 months of recent Nurse Practitioner experience preferred
