Key Responsibilities:

Conduct comprehensive annual risk assessments in patients homes Review and evaluate medical history, current conditions, and medication use Educate patients about their conditions and ensure understanding of medications Identify gaps in care, providing actionable recommendations for follow-up Complete each visit in approximately 45 minutes, with potential visits following hospital discharge or for routine annual assessments

Qualifications:

Active Nurse Practitioner license in Ohio Certified by AANP or ANCC Valid CPR (BLS) certification MSN degree from an accredited nursing school At least 6 months of current NP experience preferred
