Advance your nursing career with a vital role caring for patients in their homes. This position offers a full-time, 13-week contract for an experienced RN skilled in Home Health. Youll work Monday through Friday, 8:00 AM 5:00 PM, managing a caseload that typically involves 35 patient visits per day (30 points per week). The role is entirely field-based, providing essential care and building strong, impactful relationships with those you serve.

Key Responsibilities:

Conduct thorough OASIS assessments and develop tailored care plans Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management Monitor patient status, report changes, and communicate proactively with healthcare teams and physicians Educate patients and caregivers about diagnoses, therapy plans, medications, and preventive health practices Ensure precise and timely documentation using Home Care Home Base (HCHB) Work collaboratively with physical therapists, occupational therapists, social workers, and home health aides for comprehensive care coordination Attend case conferences, team meetings, and participate in ongoing education as needed Adhere to agency and regulatory standards for quality and compliance

Qualifications:

Active and unrestricted RN license in the state of practice At least 1 year of recent home health nursing experience, with field work preferred Proficient in Home Care Home Base (HCHB) and experienced with OASIS Reliable transportation with valid drivers license and auto insurance
