About Us Sanctuary Wadsworth is a skilled nursing and rehabilitation facility serving long-term and short-term residents with a wide range of care needs, including high-acuity residents. We're looking for an organized, compassionate team member to support our residents' social and emotional well-being and to keep their outside appointments running smoothly. What You'll Do Social Services Complete social service assessments, MDS Section sections as assigned, and care plans within required timeframes Participate in admissions, care conferences, and interdisciplinary team meetings Serve as a resident and family advocate; address concerns, grievances, and resident rights issues Coordinate discharge planning, including home health, DME, community resources, and placement Assist residents and families with advance directives, guardianship, and benefit applications (e.g., Medicaid) Monitor residents' psychosocial and behavioral needs and make referrals for mental health services Maintain accurate, timely documentation in compliance with state and federal regulations Transport Coordination Schedule resident outside appointments (physician, specialist, imaging, procedures) and arrange appropriate transportation (wheelchair van, ambulette, ambulance, family) Coordinate escort staffing when needed and communicate appointment details to nursing and families Verify transportation coverage and obtain authorizations where required Maintain the appointment/transport calendar and log; track follow-up orders and results returning from appointments Build working relationships with local transport vendors and physician offices What We're Looking For Social services experience in a skilled nursing or long-term care setting required Knowledge of resident rights and long-term care regulations Strong organization and scheduling skills; able to juggle multiple appointments and deadlines Clear, compassionate communication with residents, families, and providers Comfortable with electronic health records and basic computer applications Valid driver's license

Pay:

$46,000.00 - $55,000.00 per year
