Perform annual home health risk assessments for Medicare Advantage plan members, each lasting about 45 minutes Gather and review detailed prior and current medical histories, conditions, and medication use Educate patients about their conditions and medications to encourage understanding and compliance Identify and document gaps in care, making recommendations to promote optimal patient outcomes Focus solely on assessmentno treatment planning or prescribing required
