Monday Friday, 8:00 AM 5:00

PM Key Responsibilities:

Perform comprehensive OASIS assessments and tailor individualized patient care plans. Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management. Monitor patient progress, address changes in condition, and communicate effectively with physicians and care teams. Educate patients and families on diagnoses, treatments, and prevention strategies. Complete timely documentation using Home Care Home Base (HCHB). Foster collaboration by participating in case conferences, staff meetings, and ongoing education. Adhere to all regulatory and best practice standards for patient care and documentation. Travel daily within your assigned territory, with mileage reimbursement provided.

Qualifications:

Active RN license in the state of practice, in good standing. At least 1 year of recent field-based Home Health experience. Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS. Reliable transportation, active drivers license, and auto insurance.
