Seeking a skilled and compassionate registered nurse to deliver high-quality home health care services within a supportive contract environment. In this field-based role, you will leverage your clinical expertise to enhance patients well-being directly in their homes, providing personalized and comprehensive nursing care.

Monday Friday, 8:00 AM 5:00

PM What Youll Do:

Perform thorough OASIS assessments and develop tailored care plans for each patient Provide skilled nursing services including wound care, medication management, IV therapies, and chronic disease support Conduct 35 patient visits per day, ensuring each case meets the standard caseload expectation of 2532 points per week Educate patients and their families about diagnoses, care plans, medications, and preventive health practices Monitor patient progress and swiftly communicate clinical updates to the interdisciplinary healthcare team Maintain accurate, timely documentation using Home Care Home Base (HCHB) Collaborate with physical and occupational therapists, social workers, and home health aides Participate in case conferences, staff meetings, and relevant professional development opportunities Abide by all state, federal, and agency guidelines for documentation and patient care

What Were Looking For:

Active RN license (in good standing) At least one year of recent home health experience, including fieldwork OASIS and Home Care Home Base (HCHB) proficiency Strong clinical assessment skills and critical thinking Excellent time management and communication abilities Able to work independently with a flexible schedule Valid drivers license, reliable transportation, and current auto insurance BSN and home health compliance knowledge preferred
