In this per diem opportunity, youll perform vital home health risk assessments for Medicare Advantage plan members throughout Fulton County, including Wauseon, Archbold, and Swanton, OH. Each assessment, typically 45 minutes in length, may serve as an annual health review or a post-discharge visit. Youll help patients gain a better understanding of their health while playing a key role in closing critical care gaps.

Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan members Review and evaluate patients medical histories, current health conditions, and medication usage Educate patients on their medical conditions and prescribed medications Identify gaps in care and provide appropriate recommendations to address them No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner License in Ohio (OH) Current CPR certification (BLS) Board Certification via AANP or

ANCC MSN
