What Youll Do:

Carry out comprehensive home health risk assessments for Medicare Advantage members Review prior and current medical history, existing conditions, and medication use Educate patients regarding diagnoses and prescribed medications Spot any gaps in care and offer evidence-based recommendations Documentation onlyno need to prescribe, order treatments, or develop care plans

Requirements:

Active nurse practitioner license in Ohio Board certification from AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited nursing program At least 6 months experience as a nurse practitioner preferred Ability to travel within Fulton County to perform in-home assessments

Compensation:

Earn $80 to $90 per completed evaluation, with additional mileage reimbursement and compensation for administrative time.
