Conduct annual home health risk assessments for Medicare Advantage members Assess patients prior/current medical histories, conditions, and medication lists Provide clear education regarding medical conditions and medication adherence Identify gaps in care and advise on actionable solutions Complete visits in various Richland County locations, adapting to both annual and post-discharge assessments No responsibility for starting treatment plans or prescribing medications

Qualifications:
