Job Overview We are seeking a compassionate and dedicated Hospice Registered Nurse Case Manager to join our team and provide exceptional end-of-life care to patients and their families. Responsibilities Conduct thorough patient assessments to determine eligibility for hospice services and develop individualized care plans aligned with patient needs and wishes. Manage a caseload of hospice patients, coordinating medical, emotional, and psychosocial support in collaboration with physicians, social workers, chaplains, and other healthcare professionals. Monitor patient progress through regular evaluations, vital signs monitoring, and symptom management to ensure comfort and quality of life. Educate patients and families about disease progression, medication management, advanced directives, and available resources to empower informed decision-making. Oversee discharge planning, transition of care, and coordination with home health agencies or inpatient facilities as needed. Maintain accurate electronic health records (EHR) including documentation of patient interactions, clinical notes, medication administration, and compliance with HIPAA regulations. Participate in interdisciplinary team meetings to review patient status, adjust care plans, and ensure seamless delivery of hospice services. Qualifications Valid nursing license with current registration in the state of practice. Proven experience in critical care settings such as ICU, emergency medicine, trauma centers (Level I or II), or medical-surgical units. Prior experience in hospice or palliative care is highly preferred; home health or post-acute care experience is a plus. Strong knowledge of physiology, symptom management, infectious disease care, discharge planning, and medical terminology. Familiarity with electronic health records (EHR/EHR systems) and healthcare regulations including HIPAA compliance. Excellent patient assessment skills combined with the ability to collaborate effectively with multidisciplinary teams. Ability to manage complex cases involving behavioral health issues or geriatrics; experience with managed care policies is advantageous. Exceptional communication skills for client interaction and family education across diverse populations. Knowledge of hospital medicine practices including inpatient experience is beneficial; outpatient or clinic experience is also valued.
