Job Description

Job Overview We are seeking a dedicated and compassionate Registered Nurse to join our dynamic healthcare team. In this vital role, you will provide expert patient care across various medical settings, ensuring safety, comfort, and optimal health outcomes. Your expertise will support a wide range of clinical procedures, assessments, and treatments, contributing to the overall well-being of diverse patient populations. This position offers an exciting opportunity to work in a fast-paced environment where your skills make a meaningful difference every day. They would be supervising the aide or LPN and managing the patient cases. Responsibilities Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan, which establishes goals based on nursing diagnosis an incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan of care. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. Assumes responsibility to coordinate patient care for assigned caseload. Prepares clinical notes and updates the primary provider when necessary and at least every sixty days. Communicates with the provider (or other authorized licensed independent practitioner) regarding the patient's needs and reports any changes in the patients condition. Obtains/receives provider (or other authorized licensed independent practitioner's orders as required. Communicates with community health related persons to coordinate the care plan. Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary items and services are available.  Instructs, supervises, and evaluates home health aide care provided every 14 days. Supervision of the licensed practical nurse at least every 30 days. Other duties as assigned.
Other:
It would be a plus if they had prior home health care experience, were familiar with the OASIS data set, and had a general understanding of home health documentation requirements. They also need an BLS from the American Heart Association (AHA).
Pay:
$40.00 - $55.00 per hour
