Under limited direction, this position functions on the multidisciplinary team to coordinate patient care throughout the continuum of care and serves as a clinical resource for care management. Duty Percent of Time Cultivates and supports primary care and specialty provider co-management with timely communication, inquiry, follow-up and integration of information into the care plan regarding transitions in care and referrals. 25% Serve as the contact point, advocate and informational resource for patient families, care team, and sub-specialists. 10% Increases continuity of care by managing relationships with ambulatory care providers, transitions in care, referrals, and authorizations. 10% Communicates information and participates in satellite clinics, multidisciplinary clinics, and everyday operational functions of outpatient clinic. 15% Directly involved with the development and enhancement processes of multidisciplinary specialty clinics with the aim to improve the clinical experience for patients and families. 35% Other duties as assigned. 5%

Education:

Bachelor's degree in health-related field

Experience:

3+ years clinical experience; Pediatric experience preferred

License/Certification/Credential:
