Key responsibilities include:

Conducting annual home health risk assessments for Medicare Advantage plan members in various locations within Wagoner County, such as Wagoner, Coweta, and Porter. Evaluating patients prior and current medical history, medical conditions, and medication usage. Educating patients about their health conditions and medications during each home visit, which typically lasts around 45 minutes. Identifying gaps in care and providing recommendations to promote positive health outcomes. Focusing evaluations on assessment and educationno creation of treatment plans or medication prescribing required.

Qualifications:

Active Nurse Practitioner license within the state of Oklahoma. Current CPR certification (BLS). Board Certification (AANP or ANCC). 6+ months of recent Nurse Practitioner experience preferred. MSN degree from an accredited school of nursing.
