Locumly is seeking a dedicated Nurse Practitioner for a permanent primary care opportunity in the North Carolina region. This full-time position focuses on delivering comprehensive, patient-centered care to older adults within an innovative Programs of All-Inclusive Care for the Elderly (PACE) model. Working alongside a collaborative interdisciplinary team, you will help manage complex health needs and empower senior patients to maintain their health, independence, and quality of life.
What you will be doing:
Deliver comprehensive primary medical care to frail and elderly patients in collaboration with program physicians. Conduct thorough clinical assessments, including initial, ongoing, and acute evaluations. Formulate, implement, and manage individualized care plans within a multidisciplinary team structure. Evaluate and manage acute illnesses and chronic health conditions. Conduct home visits when necessary to ensure complete continuity of care. Support discharge planning for hospital and long-term care transitions. Participate in a provider on-call rotation to maintain continuous clinical support. Maintain accurate, timely clinical documentation in accordance with program standards.
What you must have:
Board Certified Nurse Practitioner (AANP or ANCC). Active, unrestricted North Carolina state NP license. Active DEA registration, CDS certification, and BLS certification. Minimum of 5 to 7 years of clinical Nurse Practitioner experience, with strong preference for backgrounds in geriatrics or long-term care. Experience with frail or elderly populations (or a strong interest and willingness to complete specialized program training). Must be authorized to work permanently in the U.S. without visa sponsorship.
