Job Description

Overview We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to trach patients in their homes, ensuring their comfort and well-being. The ideal candidate will possess strong clinical skills, a commitment to patient-centered care, and the ability to work independently while collaborating with a multidisciplinary team. Thia will be an 8hour shift of 3-4days a week. Duties Conduct comprehensive assessments of patients' health status, including physical, emotional, and social needs. Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers. Administer medications and treatments as prescribed while monitoring for effectiveness and potential side effects. Provide education and support to patients and their families regarding health conditions, treatment plans, and self-care strategies. Perform wound care, catheterization, and other nursing procedures as necessary. Ensure adherence to infection control protocols and maintain a safe environment for patients. Document all patient interactions accurately in the electronic health record (Epic) to ensure continuity of care. Participate in discharge planning to facilitate smooth transitions from hospital to home or other care settings. Collaborate with primary care providers and specialists to coordinate comprehensive patient care. Experience Current RN licensure in the state of practice. Previous experience in home health, geriatrics, or travel nursing is preferred. Familiarity with infection control practices is essential. Experience in primary care settings or ambulatory surgery centers is a plus. Knowledge of discharge planning processes is beneficial. Skills in infant care or emergency room (ER) settings are advantageous but not required. Proficiency in using alora plus is a plus for documentation is a plus.
