Responsibilities:

Complete comprehensive annual home health risk assessments for Medicare Advantage members Review and evaluate prior and current medical history, existing health conditions, and medication regimens Educate patients regarding their health conditions and medication therapies Identify and address any gaps in care, making professional recommendations for resolution Focus exclusively on assessment and educationno treatment planning or prescription authority necessary

Qualifications:

Active and unrestricted Nurse Practitioner licensure in Oklahoma Current CPR (BLS) certification Board certification through AANP or ANCC Minimum of 6 months recent experience as a Nurse Practitioner preferred Master of Science in Nursing (MSN) from an accredited institution Your dedication will be rewarded with competitive earnings of $80-$90 per evaluation, along with mileage stipends and compensation for administrative time.
