Key Responsibilities:

Conduct thorough annual and post-discharge home health risk assessments with visits typically lasting 45 minutes Evaluate patients prior and current medical history, chronic conditions, and medications Educate patients on their diagnoses, medication regimens, and risk factors Identify gaps in care, making evidence-based recommendations to close them Focus exclusively on assessment; no prescribing or creation of treatment plans

Qualifications:

Active Nurse Practitioner license in the state of Oklahoma Board Certification through AANP or ANCC Master of Science in Nursing (MSN) from an accredited program Current BLS/CPR certification Minimum of 6 months of recent NP experience is preferred Enjoy competitive compensation of $80$90 per completed evaluation, supplemented by a mileage stipend and additional pay for administrative responsibilities.
