Job Summary:
We are seeking a compassionate and skilled PRN Registered Nurse to join our team at TruHeart Home Health & Hospice in Norman, OK. As a PRN RN, you will provide prescribed nursing care services to patients in their homes, following a Plan of Care (POC) and according to their needs. Responsibilities:
Conduct a physical examination of patients, including vital signs, physical assessment, mental status, appetite, and diet, etc. Evaluate patients, family members, and homes to determine what health teaching will be required. Develop and implement a written Plan of Care with patients, following physician's orders and agency guidelines. Perform accurate assessments, develop and implement a written Plan of Care with patients, and adapt these actions as patient needs change. Administer medications, treatments, and IVs according to agency policy, accepted standards of care, and home health guidelines. Communicate effectively with patients, families, and physicians to ensure quality patient care. Document patient status, patient needs, nursing interventions, and patient response. Conduct admission, transfer, recertification, and discharge visits for patients. Assist with orders, assessments, wound care, lab draws, and other tasks as assigned.

Job Conditions:
Must have a driver's license and be willing to drive to patient's residence in Norman, OK, OKC, Edmond, and surrounding areas. Ability to communicate both verbally and in writing is required. Ability to access a patient's home, which may not be routinely wheelchair accessible, is required. Physical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients.
