Responsibilities:

Complete in-home health risk assessments, typically lasting about 45 minutes, on Medicare Advantage plan members Review and evaluate prior and current medical histories, conditions, and medications Educate clients about their medical conditions and prescribed medications Identify care gaps and recommend solutions Visits are non-prescriptive: no treatment plans or medication orders required

Desired Qualifications:

Active and unencumbered Nurse Practitioner License in Oklahoma Board Certification via AANP or ANCC Current CPR certification (BLS) Masters degree in Nursing (MSN) from an accredited institution At least 6 months experience as a Nurse Practitioner preferred
