Key Responsibilities:

Conduct thorough home health risk assessments for Medicare Advantage members Review and document past and current medical history, chronic conditions, and medication use Educate patients on their medical conditions and provide medication education Detect gaps in care and recommend solutions to enhance patient well-being No requirement to manage ongoing treatment or prescribe medications

Qualifications:

Valid Nurse Practitioner license in OK CPR certification (BLS) Board certification via AANP or ANCC At least six months of recent NP experience preferred Master of Science in Nursing from an accredited program This position offers competitive rates, with compensation of $80-$90 per completed evaluation, plus a mileage stipend and paid administrative time.
