Key Responsibilities:

Conduct comprehensive home health risk assessments, averaging 45 minutes per visit Travel to members homes across designated areas in Le Flore County Review and evaluate prior and current medical history, conditions, and medication use Educate members about their medical conditions and medications Identify gaps in care and recommend actions for improvement No need to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Oklahoma Current BLS (CPR) certification Board Certified through AANP or ANCC Minimum of 6 months of NP experience preferred MSN degree from an accredited nursing school Receive competitive compensation of $80-$90 for each completed evaluation, along with a mileage stipend and pay for administrative time.
