This per diem opportunity is designed for a Nurse Practitioner who values flexibility, autonomy, and patient-centered care. As part of this role, you will visit Medicare Advantage plan members in Washington County, which includes Bartlesville, Dewey, Copan, and nearby areas. Each in-home evaluation averages about 45 minutes, with visits tailored to patient needs, including annual health assessments and post-discharge check-ins.

Key Responsibilities:

Conduct annual and post-discharge home health risk assessments for Medicare Advantage members Assess prior and current medical conditions, review medical and medication history Educate patients about their medical conditions, medications, and available care strategies Identify any gaps in care and make recommendations to help close those gaps No responsibility for treatment planning or prescribing medications

Qualifications:

Active Nurse Practitioner license in Oklahoma Board Certification through AANP or ANCC Current CPR certification (BLS required) MSN from an accredited nursing school Minimum of 6 months' recent experience as a Nurse Practitioner preferred
