Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Review and document patients prior/current medical history, conditions, and medications Educate patients about their health conditions and prescribed medications Identify and report care gaps, providing recommendations for improving patient outcomes No requirement to create treatment plans or prescribe medication

Qualifications:

Active Nurse Practitioner license in Oklahoma Current BLS (CPR) certification Board certified via AANP or ANCC Minimum of 6 months' nurse practitioner experience (preferred) MSN degree from an accredited program
