Primary Responsibilities:

Conduct detailed annual home health risk assessments in members homes Review and evaluate prior and current medical histories, chronic conditions, and medication regimens Deliver clear education and counseling regarding conditions and medication management Identify care gaps and supply actionable recommendations for improved health outcomes Visits vary and may include annual assessments or post-discharge evaluations No prescribing of medication or creation of ongoing treatment plans required

Qualifications:

Active Nurse Practitioner License in Oklahoma Current CPR certification (BLS) Board Certification through either AANP or ANCC Minimum of six months recent NP experience preferred MSN degree conferred by an accredited nursing program You will enjoy competitive compensation of $80$90 per completed evaluation plus a mileage stipend and paid administrative time, all while enjoying the convenience of crafting your schedule.
