Key Responsibilities:

Conduct comprehensive annual and post-discharge home health risk assessments Review and evaluate past and present medical conditions, as well as medication histories Provide personalized education to patients regarding their health and medications Identify gaps in care and make recommendations to improve patient outcomes No ongoing treatment planning or prescription duties required

Qualifications:

Active Nurse Practitioner license in Oklahoma Current BLS certification (CPR) Board Certification from AANP or ANCC At least six months of Nurse Practitioner clinical experience preferred MSN degree from an accredited nursing program Compensation for this position is $80$90 per evaluation, along with a mileage stipend and pay for administrative time, reflecting the importance of your expertise and the value you bring to patient care.
