Job Description

Emerald Hospice is looking for a full time on call RN to join our team! The on-call RN will work 7 days on, and 7 days off to meet the after hours and weekend needs of our hospice patients. Under a physician's order and in collaboration with other Interdisciplinary Group members as described in the patient's Plan of Care, the On-Call RN is responsible for triaging calls that come into the Emerald Hospice office, providing guidance and support to patients and families who call in, as well as scheduling and making visits as needed.
The On-call RN:
Primary on call, Monday at 1630- Monday at 0830 every other week. 7 days on, 7 days off On-Call RN receives calls Hospice patients, families, caregivers, and facilities, assesses the needs of the patient and family, and coordinates needed care over the phone, makes visits as needed or requested Traveling with adequate supplies and documents to respond to anticipated and unexpected situations. Managing patients' pain and symptoms pursuant to Physician's Orders; obtaining changes in Orders as needed and updating the Patient's Plan of Care and medication list accordingly. Assessing the patient and family's need for a home visit and if so, scheduling and making the visit and notifying the patient and family, or facility of the expected visit time. Guiding, supporting, and educating to Hospice patients, families, and caregivers over the phone. Ordering durable medical equipment for the Hospice patient as needed pursuant to the physician's orders and the patient's plan of care Documenting all services provided in the patients' electronic medical record pursuant to company protocol Qualifications Graduate of an accredited nursing program. Current Oklahoma Registered Nurse license. Two (2) years' Hospice nursing experience, preferred, One (1) year of hospice experience required. Expertise in pain management, symptom control, crisis intervention, and family support. Excellent critical thinking skills and superior clinical judgment. Valid driver's license, proof of acceptable automobile insurance coverage, and reliable personal transportation. Ability to work independently, make accurate, and at times quick judgments. Ability to respond appropriately to a crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes. Competitive Pay.
