Responsibilities include:

Conducting annual and post-discharge home health risk assessments in patients' homes Completing comprehensive reviews of prior/current medical history, chronic conditions, and medications Educating patients about their health conditions, preventive care, and proper medication usage Identifying care gaps and offering actionable recommendations, all without responsibility for treatment planning or prescription writing Providing professional, compassionate care that supports patient empowerment and improved outcomes Desired qualifications: Active Nurse Practitioner license in Oklahoma Board certification through AANP or ANCC Current CPR certification (BLS) MSN from an accredited nursing program At least 6 months of current NP experience, preferably in home health or primary care Compensation is offered at $80-$90 per completed evaluation, along with a mileage stipend and administrative pay.
