Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage plan members in their residences Review and evaluate medical history, current conditions, and medication lists Educate patients on health conditions and medications Identify gaps in care and provide recommendations to address potential health risks No requirement to create treatment plans or prescribe medications

Qualifications:

Valid Nurse Practitioner license in Oregon Current Basic Life Support (BLS) certification Board Certification as a Nurse Practitioner via AANP or ANCC Minimum of 6 months current NP experience preferred MSN degree from an accredited nursing program You will receive compensation of $80 to $90 per evaluation, plus a mileage stipend and additional pay for administrative time, acknowledging your expertise and dedication to patient care.
