Deliver meaningful, patient-focused care as an experienced Nurse Practitioner specializing in home health risk assessments. This per diem opportunity provides flexibility and autonomy, allowing you to make a significant impact on Medicare Advantage plan members throughout Jackson County. Youll conduct comprehensive 45-minute in-home assessments that help shape the ongoing health journeys of individuals in Medford, Ashland, Eagle Point, Jacksonville, and nearby communities.

Responsibilities:

Perform annual home health risk assessments for Medicare Advantage plan members Evaluate and document prior/current medical history, ongoing conditions, and prescribed medications Offer education regarding disease management and appropriate medication use Identify potential care gaps and recommend next steps for follow-up Visits are assessments only; treatment plan creation and medication prescribing are not required

Qualifications:

Active, unrestricted Nurse Practitioner license in Oregon Current CPR (BLS) certification Board Certification through the AANP or

ANCC MSN

degree from an accredited program At least 6 months of current experience as an NP preferred This role offers compensation of $80-$90 per evaluation, in addition to a mileage stipend and paid administrative time.
