Visits may include annual health assessments or post-discharge checkups. Assignments are scheduled in multiple locations within Lane County, including Eugene and surrounding communities such as Springfield, Elmira, and Cottage Grove. Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Review patients medical history, chronic conditions, and current medications Educate patients on managing their medical conditions and proper medication use Identify care gaps and make effective recommendations for follow-up No requirement to create treatment plans or prescribe medications

Qualifications:
