Primary Duties:

Complete in-depth annual health risk assessments during patient home visitseach typically lasting about 45 minutes Evaluate prior and current medical history, including medical conditions and medication reviews Deliver tailored education to patients about their health conditions and medications Identify care gaps and offer actionable recommendations to improve patient outcomes No requirement for treatment planning or prescribing medications

Qualifications:

Active Nurse Practitioner license in Oregon MSN degree from an accredited school of nursing Current Board Certification (AANP or ANCC) Current CPR certification (BLS) At least six months of recent NP experience preferred This role offers competitive compensation of $80$90 per completed evaluation, plus a mileage stipend and paid administrative time.
