Essential Responsibilities:

Conduct thorough annual home health risk assessments for Medicare Advantage members Assess medical histories, review current and past conditions, and evaluate medication usage in members homes Educate patients about their health conditions and medications, fostering better understanding and self-care Identify care gaps and offer informed recommendations for follow-upno need to prescribe medications or create treatment plans Visits are approximately 45 minutes each and may take place in The Dalles, Dufur, Mosier, or elsewhere within

Wasco County Qualifications:

Valid Nurse Practitioner license for Oregon Current CPR certification (BLS) Certification through AANP or

ANCC MSN

from an accredited nursing school Six months or more of NP experience preferred
