Key Responsibilities:

Conduct thorough annual and post-discharge home health risk assessments in the patients residence Review and document prior/current medical conditions, medication history, and overall health status Offer personalized education about medical conditions and medication management Identify care gaps and provide actionable recommendations for optimal health outcomes No responsibility for creating treatment plans or prescribing medicationsfocus strictly on assessments and education

Qualifications:

Active Nurse Practitioner license in Oregon Board certification through AANP or ANCC Current Basic Life Support (BLS/CPR) certification Minimum MSN degree from an accredited nursing school At least 6 months of experience as a Nurse Practitioner is preferred You will receive compensation of $80$90 per completed evaluation, plus a mileage stipend and consideration for administrative work involved with each assessment.
