Key Responsibilities:

Perform in-home annual health risk assessments and post-discharge visits, evaluated on average in 45-minute sessions Review and assess medical history, ongoing medical conditions, and medication regimens Educate patients regarding their health conditions, medications, and best practices Identify care gaps and provide actionable recommendations to enhance patient outcomes

Qualifications:
