Core Responsibilities:

Conduct comprehensive annual home health risk assessments during home visits Review prior and present medical histories, chronic conditions, and medications Deliver clear health education to patients regarding diagnoses, medications, and care Identify and document care gaps, providing practical, actionable recommendations Visits may include annual assessments or post-discharge visits only; no prescribing or treatment plan creation

Key Qualifications:

Active Nurse Practitioner license in Oregon Board certification (AANP or ANCC) Current CPR certification (BLS) Master of Science in Nursing (MSN) from an accredited institution Minimum of 6 months recent Nurse Practitioner experience (preferred)
