Step into an impactful nursing role by supporting patients in their homes, fostering autonomy and positive health outcomes in a dynamic field-based setting. This full-time, 13-week contract opportunity offers a consistent Monday through Friday schedule from 8:00 AM to 5:00 PM. Expect a caseload averaging 3 to 5 home health visits per day, with daily travel to patient homes within your assigned territorymileage reimbursement is provided.

Key Responsibilities:

Perform comprehensive OASIS assessments and develop individualized care plans tailored to each patients needs. Deliver a range of skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management. Monitor patient progress, recognize and report any changes in condition, and communicate effectively with multidisciplinary care teams and physicians. Educate patients and their families about disease management, medications, preventive care, and ongoing treatment. Accurately document all care provided using Home Care Home Base (HCHB). Collaborate with physical therapists, occupational therapists, social workers, and home health aides to facilitate coordinated care. Participate in staff meetings, case conferences, and opportunities for continuing education. Follow all regulatory guidelines and agency standards for documentation and service delivery.

Qualifications:

Active and unrestricted RN license in the state of practice. At least one year of recent home health nursing experience, preferably in the field. Experience with Home Care Home Base (HCHB) and OASIS assessment tool required. Valid drivers license, reliable transportation, and auto insurance required.
