Schedule:

Monday Friday, 8:00 AM 5:00 PM As a home health registered nurse, you will: Complete comprehensive OASIS assessments and develop individualized care plans based on patient needs. Provide a range of skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management. Monitor, evaluate, and document patient progress; communicate promptly with the interdisciplinary care team and physicians about any changes in condition. Educate patients and families about disease processes, care routines, medications, and preventive strategies. Maintain timely and accurate patient records using Home Care Home Base (HCHB). Collaborate with therapists, social workers, and aides to ensure holistic and coordinated care. Participate in case conferences, staff meetings, and ongoing professional development. Adhere to regulatory and agency standards for both documentation and patient care.

Qualifications:

Active RN license in the state of practice, in good standing. Minimum of one year recent home health experience; field experience strongly preferred. Proficiency with Home Care Home Base (HCHB) system. OASIS assessment experience. Reliable transportation, valid drivers license, and auto insurance are necessary.
