Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in their homes Review and evaluate comprehensive medical, surgical, and medication histories Educate patients and their caregivers on medical conditions and medication regimens Identify gaps in care, document clinical findings, and provide recommendations to enhance care quality Complete thorough documentation for each 45-minute assessmentvisits may be annual wellness checks or post-discharge evaluations No requirement to write treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in the state of Oregon Board Certification via AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution Current CPR (BLS) certification Minimum of 6 months recent experience as a Nurse Practitioner is preferred Outstanding communication, organizational, and assessment skills In this per diem role, youll be compensated $80-$90 per evaluation, with an additional mileage stipend and compensation for administrative time.
