MDS Coordinator RN - Skilled Nursing La Grande Post Acute Rehab | La Grande, OR $40-$50/hour DOE | Full-Time | Relocation Support Available Join Evergreen Healthcare Group and grow your career. Overview We are seeking an experienced RN MDS Coordinator to support assessment, care planning, and clinical operations in a skilled nursing setting. This is a full-time role with a balanced mix of MDS coordination and hands-on clinical support. Schedule Structure Approx. 24 hrs/week: MDS Coordination Approx. 16 hrs/week: Resident Care Manager (RCM) support Additional floor coverage as needed (call-ins, staffing support) Key Responsibilities Coordinate MDS, CAAs, and care plans Set ARDs and manage MDS scheduling Ensure accurate documentation and regulatory compliance Collaborate with IDT and support Medicare processes Assist with unit oversight and clinical support Qualifications Active RN license (OR or ability to obtain) Skilled nursing/LTC experience preferred MDS/RAI knowledge strongly preferred EMR experience (PCC a plus) Why La Grande?
