Key Responsibilities:

Conduct thorough annual home health risk assessments for Medicare Advantage members Review and evaluate medical histories, current health conditions, and medication regimens Educate patients on their health conditions and medications to empower self-care Identify care gaps and recommend options for improved health outcomes No prescribing of medications or creation of treatment plans required

Qualifications:

Active Nurse Practitioner license in Oregon Current Basic Life Support (BLS/CPR) certification Board certification from AANP or

ANCC MSN
