Nurse Practitioner Opportunity in Asante Palliative Care! Asante is seeking a compassionate and dedicated Nurse Practitioner to join our Outpatient Palliative Care Team . Based in the heart of Southern Oregon , this position offers the unique opportunity to provide holistic, patient-centered care to individuals with serious or life-limiting illnesses in the comfort of their homes . What You ll
Do:
Deliver comprehensive palliative care in an outpatient and home-based setting. Perform thorough patient assessments , develop and implement individualized care plans , and provide symptom management. Prescribe medications , order and interpret diagnostic tests, and provide counseling and education to patients and families. Collaborate closely with a multidisciplinary team , including physicians, nurses, social workers, chaplains, and other healthcare professionals. Support patients physical, emotional, spiritual, and social needs during some of the most critical times in their lives.
